so, in order for mids/hibs to get to our keeps, they ahve to come through the milegates? is there another way in?

That is essentially true. There is a dungeon that also connects all 3 of the realm's frontiers, but people rarely come through there unless they are doing a large scale siege (like a relic raid). Reason being there are very high level mobs (65-70+) in the dungeon that you would need a large group to take down in order to make it through to another side.
